What alcohol actually does
Alcohol is one of the most normalised drugs in the world, which makes it one of the least explained. Here is what it is and what it does, without spin in either direction.
What it is
The alcohol in drinks is ethanol, made when yeast ferments sugar. Beer, wine, and spirits differ mainly in concentration, not in kind. Ethanol is a depressant: it slows activity in the brain and nervous system. The lively feeling early on isn't extra energy — it's the part of your brain that holds back going quiet first.
A "standard drink" is a fixed amount of pure alcohol, not a glass size. A large beer, a generous glass of wine, and a double spirit can each hold several standard drinks, which is why people often drink more than they think they have.
Why people drink
Most reasons are ordinary and understandable. Naming them honestly is more useful than pretending they don't exist.
- It's what's happening — parties, meals, sport, holidays, family habits.
- It makes social situations feel easier, especially if you're shy or anxious.
- It turns the volume down on stress, boredom, grief, or a bad week.
- Curiosity, taste, or ritual — the smell of a particular drink, the end of a shift.
- Because stopping feels harder than continuing. For some people, that's where it lands.
None of these make someone weak or foolish. But the reason matters, because alcohol used to manage feelings tends to ask for more of itself over time.
What happens when you drink
First 10–20 minutes
Alcohol passes from the stomach and small intestine into the blood, and reaches the brain. Judgment and inhibition loosen first — which is why people often feel lighter and more talkative.
30–90 minutes
Blood alcohol usually peaks. Coordination, reaction time, and memory formation are measurably worse, often before someone feels drunk.
A few hours in
The body clears roughly one standard drink per hour, and nothing speeds that up. Speech, balance and decision-making stay affected until it's cleared.
Overnight
Alcohol suppresses deep and REM sleep, so a long night's sleep still leaves you unrested. Dehydration, low blood sugar and a rebound in brain chemistry cause the next-day fog.
The next day
Mood often dips: anxious, flat, or irritable. This is a physical after-effect, not a personal failing.
In the brain
Alcohol boosts GABA (the brain's brake) and dampens glutamate (the accelerator). That's the slowing, the slurring, and the gaps in memory — the brain stops writing things down before it stops talking.
It also triggers dopamine, the signal your brain uses to mark something as worth repeating. Repeated often enough, the brain adapts: it expects the alcohol and adjusts its own chemistry around it. That adaptation is tolerance, and it's the mechanism behind needing more for the same effect.
In the body
- Liver: processes almost all of it, and takes the strain first. Damage builds quietly and often without symptoms.
- Heart and blood pressure: regular drinking raises blood pressure and disturbs heart rhythm.
- Sleep: less deep sleep and less REM, even after a long night.
- Stomach and gut: irritation, reflux, nausea.
- Long-term risk: alcohol is classified as a carcinogen, and is linked to cancers of the mouth, throat, oesophagus, liver, bowel and breast.
- Developing brains: the brain keeps maturing into the mid-twenties, and alcohol affects it more during that time.
